**The St. Johns County Volunteer Application is an application for clearance to be on school campus. All parents who will attend parent-teacher conferences, student performances, lunches with their children, etc, must complete an application and be cleared to be on campus.**

Patriot Oaks Academy loves volunteers! While there is no obligation attached to your registration, we certainly hope you will choose to make yourself available to the school in some capacity. Every hour makes a difference in our school and the lives of our children!

If you are interested in volunteering for a PTO committee, in addition to filling out the form for the county, please click the button below to submit an interest form to the PTO.  For more information, visit the PTO Committee & Event Volunteers page on the website.

You must be an approved volunteer in the St. Johns County system to enter our school beyond the front desk. This is for the safety & security of our students and staff.

This includes any of the following:

  • Parent-Teacher Conferences
  • Lunch with your child
  • Walking your child to class the first day of school
  • Field Trips
  • Helping teachers with class projects
  • Assisting with PTO sponsored events
  • Volunteering in the classroom

We encourage all parents & guardians to register as a volunteer! Please note, the process to clear applications can take up to six weeks. All parents, guardians & family members who will want to be on campus at any point during the school year should fill an application out now.

FREQUENTLY ASKED QUESTIONS

WHO SHOULD SUBMIT AN APPLICATION FOR A BACKGROUND CHECK?

Anyone wanting to come to the school grounds should apply for clearance. This includes parents, grandparents, & community members. Don't forget about relatives who may be out of town but will come to visit and want to have lunch with your child. Have them fill out the form now.

WHY DO I NEED TO SUBMIT THIS FORM?*

The county requires that you submit an application for an official background check conducted by the St. Johns County Sheriff’s Office and the National Sexual Offenders Database. If you do not submit this form for clearance, you may be denied admittance to the school for the protection and safety of our children.

HOW LONG DOES THE APPLICATION PROCESS TAKE?

Completing the application and submitting it for review takes less than five minutes. Once submitted, the application can take up to six weeks to be approved/cleared. For this reason, we recommend all parents and family members who anticipate wanting to participate in events on campus complete an application as early as possible.

WHAT INFORMATION DO I NEED TO HAVE TO APPLY?

All forms must contain your full legal name including middle name (no nicknames), address, phone, gender, birthdate, and social security number. If you do not have a middle name, please enter “NMN” in the Middle Name field in the application. If you are here on a visa and/or you do not have a social security number, please contact the St Johns County School District Volunteer Services at 904-547-3945 for further instructions.

HOW WILL I KNOW IF I AM APPROVED?

If you have a time-sensitive situation (an upcoming field trip, school activity, etc), you may contact the Volunteer Coordinator two weeks after you have submitted your application. She will be able to give you an update at that time. Otherwise, you will know the status of your application upon arrival and check-in at the school. The average time for an application to be approved is six weeks, so please plan accordingly.

If there are any issues with your application, you will receive a phone call from school administration.

IF I COMPLETED THIS PROCESS, WHAT DO I NEED TO BRING TO SCHOOL TO CHECK IN?

Every person arriving on campus should present a photo ID with a name that matches the name that was submitted on the Volunteer Application through the county. Approved forms of identification are government issued photo ID, Drivers License or valid passport.

If you are a legal citizen of the United States and you do not have a driver’s license, you should contact your local DMV to secure one in advance of coming to school.

Upon check in, you will be given an authorized visitor sticker that you must wear in a visible location on your person the entire time you are on campus. If you do not have a visible visitor sticker, you may be asked to return to the front office to check-in and receive one.

DID I ALREADY DO THIS?

Your background check is active for 3 years. If you are unsure of whether or not you have a valid background check on file, please contact the Volunteer Coordinator via email to check the system prior to submitting another application.

If you have done this for another St. Johns County school within the last 3 years, you do NOT need to resubmit. If you are transferring to Patriot Oaks Academy from another St Johns County School, you will need to email our Volunteer Coordinator so she can add you to Patriot Oaks Academy. The email must include your full legal name (first, middle, last & suffix) that was used to register in the system as well as the date of birth (month/date/year). Volunteer clearance does *not* transfer between Florida counties, only within St Johns County School District.

*By completing a School Access Form you are agreeing to a criminal background check and a sexual offender check. You are also agreeing to abide by the policies and procedures of the St. Johns County School Board and its facilities while volunteering, Your personal information will be kept confidential and you are expected to keep any and all information you may see or hear while volunteering confidential. The St. Johns County School District reserves the right to restrict and/or deny a person’s volunteer privilege when deemed necessary.

If you have any additional questions, please contact our Volunteer Coordinator:

Meredith Connell, PatriotOaksVolunteers@gmail.com

Important: In the event your check bounces for any event/item, you will be required to pay the original amount required and the bank fee within 48 hours.
